What specialists want in a referral
A specific question, why it cannot be answered where you are, the history and results that bear on it with dates, what has been tried and how it went, current medications and allergies, and how urgent it is. Attach full records rather than pasting everything into the letter.
State urgency honestly and say why. If a finding needs faster review than a routine referral allows, such as rest chest pain or a new neurological deficit, a phone call to the specialist or emergency care may be the right step, not a letter.

Should I include the whole history?

What if I need urgent input?

Will it add clinical details?
No. It uses only your notes and marks anything missing, such as an allergy list or a test date, for you to add.
Can I include patient identifiers?
